An intake form will be emailed to you after scheduling. Please complete promptly — there is a field to accept appointment reminders via text messaging and without your consent, you will not receive these notifications. There is a $50 charge for missed appointments.

Insured clients: Select “medical massage” if you are planning on using your insurance. An intake form will be emailed to you with fields for your insurance information. Please complete the intake form promptly and include clear front and back photos of your insurance card. Allowing sufficient time to verify benefits and obtain prior authorizations is a client responsibility; appointments may be rescheduled if information is not provided in time.
