Hello, and thank you for booking online! I see both insured and private pay clients. I am in network with MODA and BCBS/Regence. I also accept Motor Vehicle Accident claims. PROVIDENCE CLIENTS: Due to an abrupt change in Providence policy regarding massage coverage, I am accepting “self-insured” Providence clients only. Please contact me directly so I can look up your policy and determine the specifics of your coverage.

New private pay clients: an intake form will automatically be emailed to you once your booking is complete. Please fill this out prior to your first appointment.

New insured clients:. Select “Medical Massage” if you are planning on utilizing your insurance. An intake form will be emailed to you with fields for your insurance information. Please complete the intake form promptly and include front and back photos of your insurance card. Allowing sufficient time to verify benefits and obtain prior authorization is a client responsibility; appointments may be forfeited if information is not provided in time.
